Full Job Description

To support individuals, their families andcarers, and the bereaved (all of whom are referred to as patients) who havebeen affected by a diagnosis of cancer.

To liaise with local GPs, Hospitals, andservice providers to promote The Trust and its services.

Main duties of the job

1. To welcome, interview and assess the holistic needs of patients in accordance with professional guidelines, to devise a plan of evidence based care, to implement and evaluate plans and to maintain records in respect of each.

To refer to other agencies as appropriate

2. Work collaboratively to provide advice and support to patients including carers and the bereaved using effective and appropriate communication skills

liaise with The Therapists and be responsible for monitoring and managing the provision of therapeutic treatments to meet the physical and emotional needs of patients, including contributions to regular patient reviews

instruct and educate patients, carers and families on a range of therapeutic activities walking for health

5. To promote the work of The Trust to local groups and organisations, together with local GP practices and the cancer hospitals supporting patients from The Trust, by visiting to give talks and presentations and by sharing of information and maintaining contact, 1.Towelcome, interview and assess the holistic needs of patients in accordance withprofessional guidelines, to devise a plan of evidence-based care, to implementand evaluate plans and to maintain records in respect of each.

3. To undertake, as required, home and / orhospital visits and work from the community hubs if required.

4. To liaise with the therapists andcounsellors and be responsible for monitoring and managing the provision of treatmentsto meet the physical and emotional needs of patients, including contributionsto regular patient reviews.

5. To identify and report to the Chief Executiveand / or Senior Nurse any performance issues relating to therapeutic and / orcounselling staff.

6. To instruct and educate patients, carersand families on a range of group activities , closer to nature, yoga.

7. To work as a flexible team member andparticipate in staff meetings.

8. To creatively and efficiently utiliseavailable resources in the best interests of the patients and The Trust

9. To maintain accurate records and up todate statistical information

10. To support students on placement andvolunteers supporting the work of The Trust

11. To share as appropriate along withother staff daily or routine tasks required for the day to day running andoperation of The Trust and its premises.

12. To be familiar with and abide by TheTrusts organisational and employment policies.

13. To work within legal frameworks andprofessional guidelines

14. To act as a role model, seeking tomaintain the highest standards.

Essential

  • NMC First Level registered nurse

  • In depth PQE, to include experience within speciality at Band 5


  • Desirable

    A qualification and / or experience in cancer and palliative care
