Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services - and to prevent them becoming disenfranchised and isolated., Responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care without supervision.

To assist in leading and monitoring the performance and work of the nursing team.

To deputize for the Unit Manager as and when required.
