Physiotherapist - Forest Neighbourhood Team

Working as an autonomous practitioner within an integrated team you will provide timely and expert professional assessment and treatment interventions to patients either in their own homes or other community settings. This will be achieved through a combination of hands on expert clinical skills and knowledge as a Physiotherapist to treat, educate and support patients and by working with patients and their families to assess, plan and implement appropriate care plans and treatment. Provide case management for a designated caseload of patients who have immediate and / or complex care needs. Triage patients to ensure prioritisation of clinical need. Develop and co-ordinate individualised rehabilitation plans in close liaison with the team, the patient and their carers as well as all other services. Work holistically with all team members, carrying out joint visits and actively participating in the integration of services - increasing the knowledge of all team members through case
