Full Job Description

The Coordinator will work with the specialist multidisciplinary team to ensure the effective co-operation in the delivery of the diabetes service across SATH and Shropshire Community Health Trust. they will develop administrative systems to streamline the patient journey and improve the quality of patient care.

The co-ordinator will actively seek to ensure that all patients are treated within national Best Practice Tariff targets set for diabetes patients.

Where these targets are not likely to be met; to proactively take steps to ensure that patients treatment schedules are brought back within target.

To understand the pathway of each individual patient; monitoring their progress and ensuring timely intervention is achieved.

Develop information systems, in line with Trust strategy, for the recording and reporting of clinical and performance data. Working across multiple systems, ensure that the key clinical and administrative data is recorded and validated.

Main duties of the job

1. Co-ordinate the clinical pathway for diabetes patients.
2. The post holder is expected to co-ordinate, organise and modify multiple clinical interventions and activities for each patient, across directorates and multiple clinical services; and respond to variations in the plan accordingly.
3. For each individual patient, to understand current and future requirements; and then to take proactive steps through current action and forward planning, to ensure this pathway is maintained.
4. To challenge, as appropriate, the progress of patients' journeys with all members of the diabetes MDT (Consultants, Nurses, Dietitians, Psychologists etc).
5. To persuade multiple clinical service providers to alter existing appointments as necessary to ensure diabetes patient schedules are met.
6. To actively seek to alter patient appointments at own discretion through communication and instruction to service areas and communicate alterations as appropriate.
7. Deal with non-clinical queries as necessary.
