Full Job Description

Thames Reach's Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support., Assess the support needs of new clients in a person-centred manner, using a standardised process.
- Risk assess clients dynamically and participate in risk mitigations and safeguarding work.
- Work alongside various partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
- Represent the team at multi-agency meetings and case conferences.
- Build relationships with external service providers, to improve the diversity of support offered to clients, including those with no recourse to public funds.
- Work with clients who present with a variety of support needs, including people who are fleeing domestic violence and require support and signposting to specialist services whist working across a variety of databases to accurately record your work.

Excellent communication, interpersonal, and advocacy skills, proven within a context of working with vulnerable adults.
- The knowledge and understanding of relevant support strategies to manage a caseload of 15-20 clients.
- Strong time management and prioritisation skills, and the ability to deploy these independently.
- The ability to deeply listen to our clients and put them in control of their own recovery.
- Have a creative approach to resettlement work, and be able to work imaginatively with clients to build independence.
