Full Job Description

This role is based at Broomfield Hospital, Chelmsford working as part of the Social Prescribing team, supporting hospital discharges from Broomfield Hospital. The main duties includeresponsibility for referrals, onward referral to other services, provide ongoing support which could include telephone calls, assistance at appointments, maintenance of databases, provide relevant reports and maintain the CRM database, attending meetings, undertaking home visits/meeting citizens in their home after being discharged from hospital/follow up calls (this list is not exhaustive). You will work with individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.
