Full Job Description

As Lead cancer support worker, you will support teams to provide regular supported conversations with the person living with cancer, assessing their holistic needs, and supporting them to self-manage by providing relevant information and advice, and signposting to other resources and services available to meet their identified needs To guide and educate teams to follow clear pathways to achieve effective holistic needs assessment. To guide and educate teams to further support and manage patients at all stages of their cancer pathway; ultimately to optimise patients access to the right help at the right time, providing a seamless experience of care. To support the implementation of new cancer care pathways such as stratified follow up/RMS. Line management responsibility of the Cancer Support worker team including the supervision, recruitment, appraisal, performance and the utilisation, delivery and adherence to trust policies and procedures. This will be in collaboration with the CNSs that
work directly with the site specialist cancer support workers. To encourage the development of any staff directly managed to their full potential through systematic individual performance development review and the establishment of personal development plans, reflecting the needs of the organisation and the individual.
