ENHANCED PRACTITIONER NURSE

A new opportunity has arisen for a full time Enhanced Practitioner Nurse. We are looking to recruit a nurse who is committed to delivering the best care to our patients.

Primarily working with the Wellingborough & District PCN nursing team, GPs and practice nursing teams within the network Practices. you will continually review and evaluate services and work with the management team in the implementation of change., As an Enhanced Practitioner Nurse, you will form part of a PCN team providing care including autonomously, for our patients mainly within their own homes or a care home settling. This will include review and management of chronic diseases, review of CCT patients and their care plan, assessment and management of common minor illness complaints, annual LD reviews and other community based services such as covid and flu vaccination. You will demonstrate safe clinical decision-making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Wellingborough & District Hub comprise of six practices supporting a registered practice population in Wellingborough of approximately 78,000.
